GOVERNMENT FUNDED SUPPORT AT HOME PROGRAM
Support at Home Program (SaHP)
… is for those who the My Aged Care Assessor determines to need a higher level of care to remain independent at home. A Centrelink Income Tested Fee form (part pensioners and self-funded retirees) must be completed to assess income to determine your co-contribution to your home care package. Moving forward in the Support at Home Program (SaHP) all pensioners must co-contribute to Everyday Living & Independence Services. You will need to provide Centrelink with an Income Assessment Form outlining your financial details and they will then assess and advise you on the contribution you need to make.
Full and part Pensioners & self-funded retirees are required to co-contribute towards their package.
There are eight levels of care under the Support at Home Program, and a maximum subsidy is available on each.

7am – 7pm- $105.80 per 30 minutes

Domestic Service
WEEKDAYS
7am-7pm- $95.00 per hour

weekends by negotiation when required- $186.00 per hour
- Note: Nursing Services not scheduled over weekends or public holidays. Private bookings can be arranged.
Other Fees
Care Management Fee
- Charged against the Support at Home. Coordination of assessments, care planning, reviews, referrals, scheduling and 24hr point of contact.
Co-Contribution fee
- Assessed by Centrelink for full, part pensioners and self-funded pensioners. This is to be paid by the client when an invoice is issued.
Funded Packages Ts & Cs
- Spring Clean – Arranged by Infinity Home Care at reviews to determine the tasks required.
2. Travel – incurs a fee of $1.20 per kilometre for all travel during service times
3. Clients living 20km or more from the Infinity Home Care Office, travel time and kilometres for staff will be discussed at service commencement.
4. To avoid being charged a cancellation fee we require 24 hours’ notice or more of a cancellation to an agreed service.
5. Fees and charges – reviewed annually, we will provide four weeks’ notice of any changes to fees and charges. All of the above Infinity rates are GST free
